Verify Your Benefits
Please follow these instructions to determine your eligibility:
- Fill out the attached forms, "Patient Information" and "Benefits Assignment" in their entirety, as this information is necessary for your insurance company, to be provided accurate information in order for your claim to processed in a timely fashion. Please ensure that you sign the forms in order for our billing service to be submit your claim to your insurance company. Without your signature, we will be unable to file your claim and unfortunately, you will be responsible for the balance.
- Please create a clear photocopy of the front and back of your insurance card, and a copy of your driver's license. The importance of your attaching a copy of the front and back of your insurance card is so that our billing company has accurate information, such as, your identification number, group number and the correct address to mail your claim for expeditious processing.
- Please fax your completed forms to: 561.881.2168
- If you are with blue cross/blue shield, (of any state), you will be reimbursed directly by your insurance company.
Schedule Your First Appointment
After benefits have been verified (which can take several business days), we will call you back to schedule your initial appointment. You can expect a thorough intake session before your treatment begins, and together we will formulate a treatment plan that best suits you.
